RFP Price Escalation Clauses > Best Practices

We are looking for best practices around price escalation clauses in contracts.  Suggestions could include, but are not limited to, tying price escalations to an index, establishing a base selling/purchasing price, frequencies for adjustments, etc.  On the one hand, we are looking for confirmation on how we address price escalations and on the other out-of-the-box ideas for dealing with this fact of business life.
